That's how I look at it, too. It's not "OMG someone died," it's a matter of how frequent such fatalities or injures are. And whether they occur in the normal course of activity or primarily when someone's doing something stupid. That aligns with what I know as an engineer, too. Machines are designed with safety factors (multipliers on numerical ranges stated, like a weight limit for an elevator) and often overlapping safety mechanisms. The part that does worry me is if "someone" doing something stupid is one of the operators. Like, what if safety equipment is malfunctioning, or a safety protocol is not being practiced, and a person being paid the prevailing minimum wage doesn't care enough to notice.
